Make a Booking

Please complete the booking form below for either a free Clinical Appointment with a trained clinician (for ages 4 years old and older) or a Formal Autism Assessment. When you select the service that you would like via the drop down menu, click continue and a calendar will appear. Please select a date and time that is convenient for you and click continue. Once you have filled in the form, you will receive a confirmation email with details of your booking.
